Queenvision Health Services, LLC will provide Case Management Services through the EPD Waiver Program administered by the District of Columbia Department of Health Care Finance.
Our case management services include:
Evaluating the needs, preferences, and goals of each beneficiary to determine appropriate services and supports.
Working collaboratively with beneficiaries and their support networks to create a person-centered service plan.
Coordinating Medicaid waiver services, healthcare providers, and community resources.
Ensuring services are delivered according to the Individual Service Plan and that beneficiary needs continue to be met.
Advocating for beneficiary rights and assisting individuals in accessing available services and programs.
