Individuals We Support

We provide case management services for:

• Older Adults (Ages 65 and Older)
  Seniors who require assistance to maintain independence and access essential healthcare and community resources. 
• Adults with Physical Disabilities (Ages 18–64)
  Individuals living with physical limitations who need support coordinating medical, personal, and social services. 
• Medicaid-Eligible Residents of Washington, DC
  Individuals enrolled in or eligible for the DC Medicaid and receiving services through the EPD Waiver.
